Step 1

Open the Scripts panel as follows:
InDesign CS5.5 and CS5: Go into the Window menu and choose Utilities > Scripts.
InDesign CS4 and CS3: Go into the Window menu and choose Automation > Scripts.

Step 2

Double-click the script in the Scripts panel.

Input your dimensions and any other options needed. Then click OK.
(The inches version is shown below, but there are versions for picas and millimeters as well.)
